Understanding Dental Insurance
Smith Family Dentistry is a participating provider with many dental insurance carriers. It is the patient’s responsibility to determine if our office is in-network with your particular dental plan. Please remember, regardless of use of dental insurance, the patient is responsible for full payment of services provided.
Dental insurance is a contract between the patient and the dental insurance carrier. As a courtesy to our patients, Smith Family Dentistry will file your dental insurance claim and accept “assignment of benefits”. Every patient that files a dental claim will receive an “Explanation of Benefits” from his/her dental carrier. The patient is responsible for the charges that state “patient responsibility” and any other fees that may apply. Please note that all services are not a covered benefit by dental insurance plans. Any service that is provided by our office and is not covered by the dental plan is the responsibility of the patient in its entirety.
